DECLARATION OF CONSENT
ON THE PROCESSING OF PERSONAL DATA
By filling in the data and sending it to Ascendis I agree with:
•processing my data for consumer-related research and analysis;
•receiving marketing information related to Ascendis products,
OR
a) I agree that my personal data consisting of:
(i) name and surname,
(ii) email address,
(iii) phone number,
hereinafter referred to as "Data”,
to be processed by ASCENDIS CONSULTING S. R. L., with Registered office in Bucharest,
Sector 1, Str. ION IONESCU DE LA BRAD no. 2b, "Gabrielle Center" Office Complex, ground
floor, section I, Room 2, registered with the Trade Register under no. J40/3140/1997, VAT number
9398288, hereinafter referred to as ”The Company” or ”Ascendis”,
according to (EU) Regulation no. 2016/679 on the protection of individuals with regard to the
processing of personal data and on the free movement of such data and repealing Directive
95/46/EC (General Data Protection Regulation);
b) I expressly consent for the Company to process the Data in accordance with the General
Data Protection Regulation
c) I agree that the processing of the Data by the Company will be carried out as follows:
(i) Legal ground: consent of the Undersigned;
(ii) Purpose of processing: the transmission by the Company of periodic information on
news about the Company's activity, organization of events, offers of services offered by
the Company, etc.
(iii) Duration of processing: as long as necessary according to the requirements of the law
(iv) Recipients of the Data: The Company, the commercial partner of the Company
empowered to transmit the information.
(v) Data transfer: Data may be transferred to third parties for the purpose referred to in let.
ii) above, without the prior consent of the Undersigned. I have taken note of and
expressly accept this right of the Company to transmit this data.
(vi) Access to Data: the Data will be accessed by the employees of the Company only for
the purposes referred to in let. ii) above, these persons being obliged to maintain the
confidentiality of the Data and being trained on Data protection.
(vii) The consent is given freely, the processing is not conditioned by the execution of a
contract or the provision of a service;
(viii) I have been informed of the Company's obligations regarding the Data, namely
that the Company undertakes:
• to process the Data according to legislation in force;
• to confirm to the Undersigned the processing or not of the Data as a result of a
written request addressed to the Company;
• to rectify, update, block, delete or transform into anonymous data, free of charge,
the Data whose processing does not comply with legal provisions;
• to cease the processing of Data that is not processed in accordance with the legal
provisions, free of charge;
• to adopt all necessary security measures to protect the Data against unauthorized
access to the Data;
• to facilitate the exercise of the Undersigned's rights with regard to the Data;
• to notify competent authorities in the event of a Data security breach;
• to inform me of the breach of Data security, if the breach is likely to generate a high
risk to my rights.
(ix) I have been informed of the Undersigned's obligations with regard to the Data,
respectively, I am obliged:
• to provide true, accurate and complete Data. If the Data is not true, accurate and
complete, I understand that I have an obligation to notify the Company as soon as
possible in order to remedy this situation;
• to update the Data, when the situation requires it, to be true, accurate and complete.
(x) I have been informed of the rights of the Undersigned with regard to the Data,
namely:
• the right of access to Data, namely: i. to obtain from the Company a confirmation
that data concerning me is being processed or not; ii. to receive from the Company
a copy of the processed data;
In order to exercise the right of access, I understand to submit a written request, dated
and signed, to the Company, at the address mentioned in the preamble hereof, for the
attention of office@ascendis.ro;
• the right to rectify Data, namely to obtain the completion of Data that is incomplete;
• the right to delete Data that exceeds the mandatory data to be provided for the
aforementioned purposes;
• the right to withdraw consent at any time. Withdrawal of consent does not affect the
legality of the processing carried out on the basis of the consent before its
withdrawal. Before giving consent, the undersigned was informed of this. The
withdrawal of consent is made by sending an e-mail to the address:
office@ascendis.ro.
• the right to restrict Data processing;
• the right to Data portability, namely the transmission of Data directly by the Company
to other entities, at the request of the Undersigned, if this is technically feasible;
• the right to object to the processing of Data that exceeds the mandatory data to be
provided for the aforementioned purposes;
• the right to contact the competent authorities if I consider that the Data processing
